We hope your first visit with Elite Women's Care will be the beginning of a long-lasting relationship with you and your family. We want to get to know you and your medical needs to help you stay as healthy as possible. New patients and patients who have not been to our practice within the past year are requested to arrive 15-20 minutes early to complete the registration process. New patients are also encouraged to call their insurance carrier to verify that our physicians are currently participating with their specific insurance plan. Please bring the following to your first appointment:
- All medications you are currently taking
- Your current insurance card
- Photo identification
- A list of allergies to medications and/or dyes
- A referral from your primary care physician, if required by your insurance plan
- Payment for any required co-payments, deductibles, non-covered services, or down payment arrangements
Please remember that if your insurance plan requires you to have a referral to see an OB-GYN, you must make certain that we have that referral before your visit. Otherwise, your appointment will be rescheduled or you will need to assume financial responsibility for the services provided.
